TALENT AUTHORIZATION AND RELEASE AGREEMENT

I acknowledge that I may be selected by GreatCall and/or Jitterbug as a potential participant and that my story and/or likeness may be used in TV, Radio, Print and Online materials. In the event I am chosen to participate, and my name, voice, and/or likeness is utilized in the Campaign, then in consideration of the potential payment to me of the appropriate talent guild minimums in each category of use, I hereby agree as follows:

I hereby grant my consent, authorization and agreement to GreatCall and/or Jitterbug, and its clients, employees, agents, successors and assigns, the right to take pictures of myself, alone and with others, for still photographs, motion pictures and television, including video-tape, and to utilize such pictures in any manner desired by GreatCall and/or Jitterbug for publicity, advertising or promotion purposes. I further agree, authorize and grant GreatCall and/or Jitterbug the right to use my name, physical likeness, recordings of my voice and all instrumental, musical or other sound effects produced by me, in connection with such publicity, advertising and promotions. All rights and such pictures, recordings, publicity, advertising and promotion material shall belong solely to GreatCall and/or Jitterbug and I shall have no right or interest therein.

I fully and forever release and discharge GreatCall and/or Jitterbug of and from any and all claims, demands, actions, causes of action, suits, controversies and all liabilities of every kind of nature accruing to me and arising directly or indirectly from the utilization of voice and all instrumental, musical or other sound effects produced by me, in connection with such publicity, advertising and promotions conducted by GreatCall and/or Jitterbug. That without limiting the generality of the foregoing, I agree and acknowledge that the foregoing release shall include any and all rights under California Civil Code S3344 and any claim for invasion of privacy relating to such publicity, advertising and promotions.

I hereby certify and represent that I have read the foregoing and fully understand the meaning and effect thereof, and intending to be legally bound by the statements made herein.
